Accutadyn (Isotretinoin)

Accutadyn (Isotretinoin) It is a powerful oral retinoid chemically derived from the vitamin A. It was developed specifically to treat the most severe forms of acne and is one of the most effective medications ever introduced in dermatology.

Introduced at the end of the 20th century and approved by the FDA in 1982, isotretinoin has revolutionized the treatment ofsevere and treatment-resistant nodular-cystic acne, becoming the gold standard of treatment for patients who do not respond to antibiotics, topical retinoids, or other conventional therapies.

Thanks to its effectiveness, Accutadyn is now widely used in cases where acne causes risk of permanent scarring or significant psychological impact, significantly improving patients’ quality of life.

How Isotretinoin works

The high efficacy of isotretinoin stems from its action on all the major pathogenic mechanisms of acne: the

Reduction in sebum production

Isotretinoin drastically reduces the activity and size of the sebaceous glands, decreasing sebum production by up to 90%. This eliminates the main substrate necessary for the proliferation of Propionibacterium acnes

Normalization of keratinization

The drug regulates the cell renewal process within the hair follicle, preventing pore blockage and the formation of microcomedones, which are precursors to more serious lesions.

Anti-inflammatory action

It reduces the production of pro-inflammatory cytokines (such as IL-1 and IL-8), decreasing redness, swelling, and pain associated with severe acne.

Long-term effect

One of the main benefits of isotretinoin is the prolonged remission: Many patients experience complete or nearly complete resolution of their acne after just one course of treatment, with results that last for years.

Clinical efficacy

Comparative studies have shown that isotretinoin is more effective compared to combinations of antibiotics and topical retinoids in reducing:

  • superficial inflammatory lesions

  • of deep inflammatory lesions

  • of non-inflammatory lesions

The results were independent of age and duration of acne, confirming the effectiveness of the treatment in different types of patients.

Other uses and off-label use

Dermatology

In addition to severe acne, isotretinoin can be used (in selected cases) for:

  • severe rosacea

  • severe seborrheic dermatitis

  • hidradenitis suppurativa

Off-label use in bodybuilding

In bodybuilding, it is sometimes used to treat acne caused by anabolic steroids. However, this use involves significant risks, especially with regard to liver function and lipid levels, and requires extreme caution.

Dosage and method of administration

  • Initial dosage: 10–20 mg per day

  • Standard dosage: 0.5–1 mg/kg/day

  • Durable cases: up to 2 mg/kg/day (under medical supervision)
